AB After  a  healthcare  professional  examines  the  patient  and  documents  the  examination  and  evaluation/assessment,  the  next  step  is  determining  the  Plan  of  Care.  This  process  includes  two  parts:  (1)  documenting  Expected  Outcomes  and  Anticipated  Goals  for  the  patient  (Chapter  19),  and  (2)  documenting  the  Planned  Interventions  to  help  the  patient  achieve  the  expected  outcomes  (Chapter  20).
