AB While  writing  a  book  appears  to  be  a  solitary  endeavor,  it  is  not.  There  are  many  individuals  who  contribute  to  a  book  directly  or  indirectly.  Throughout  my  career,  my  patients  have  helped  me  learn  how  to  be  a  more  effective  practitioner  and  to  consider  each  as  an  individual  and  not  to  make  assumptions  about  goals  based  on  age  or  disability.  To  all  my  patients  over  the  years,  I  owe  a  debt  of  gratitude,  and  much  that  I  have  learned  from  them  is  part  of  this  book.
